Check whether the home, support and provider feel right

A home and living arrangement needs to work in everyday life. That means thinking about the home, support needs, housemates, provider, environment and daily life.

When you are considering options, it is natural to start with the home. It is also important to ask about the support, communication, continuity and what happens if needs change.

Identitywa works with individuals, families and support networks to understand what matters before a decision is made.

The home and housemates

Consider location, accessibility, bedrooms, bathrooms, shared spaces, feeling comfortable at home, compatibility and whether the environment feels right.

The support

Consider daily life, communication needs, support worker capability, interests, hobbies, activities, appointments and community access.

The provider

Consider stability, registration, communication, continuity, experience, quality processes and what happens if needs change over time.

What does good support really look like?

Good support includes what you see day to day, and the things that may not be as obvious at first. This checklist is designed to help you understand what good support can include and what to ask before choosing or reviewing a provider.

Daily life

What does a typical day look like?

Communication

Who is the main point of contact, and how does the provider communicate with families and support networks?

Person-centred support

How are interests, hobbies and activities supported?

Changing needs

What happens if needs change over time?

Provider confidence

How does the provider demonstrate stability, quality, communication and continuity?
