Supports for Adults

As we journey through out adult life, we often glimpse new paths and broader horizons for ourselves. Some well-placed supports are sometimes all we need to set us on our way.

Our Planners will help you find the right mix of supports for your chosen path.

Sharing the journey

Our Planning Officers can help you choose a range of living and community supports tailored to suit your lifestyle:

Future Planning

We’ll work with you to plan a range of supports for today and the future.

We’ll help you explore funding options and ways of managing that funding.

Work Alternatives

At Identitywa, we understand the importance of participating in your community with a sense of purpose. Our staff are committed to helping you explore work or work alternatives suited to your interests and skills.

Home Sweet Home

Finding the right place to call home gives us security and comfort. Our Planning Officers can help you explore living options in an individual or shared setting, with support tailored to individual needs and personal goals.

Fun and friends

Our support workers make it easy for you to take part in activities you would like to do. Music, friends, sport – whatever your interests, we’ll help you make them a regular part of your life.

Community connections

Making connections with our local community can expand our social networks and deepen our sense of belonging. Identitywa staff will support you to make meaningful community connections. A walking group, pottery class or a shared love of animals – that part is up to you.

Change of scene

A short holiday break can do wonders to lift our spirits. If you’re in need of a change of scene, we can arrange a short break for you, your whole family, or a group of families together.

Family wellbeing

We also have a range of supports for carers. Our Houses provide a homely environment where your family member can stay for a few hours or a few days. Our support workers can also provide support in the family home.

Seven days a week, day or overnight – there are plenty of options to give you time to focus on your own needs. We’re also keen to discuss other ways we can support you in your caring role. Explore ideas with our Planning Officers.

We look forward to speaking with you soon.
