Sharing the journey

Our Planning Officers will help you choose a range living and community supports tailored to suit your family’s lifestyle.

At Home

We can offer help with the activities of your child’s daily life in your home, or facilitate the goals and outcomes from developmental learning programs from your therapy provider or school. We support your child’s needs, with the advantage of providing your family with a break also.

Having a Break

This may be in your home, local community or in one of our Children’s (Respite) Houses where we will continue with planned activities or learning programs.

Fun, Friends and Living

We can support your child with their interests and passions, helping to build friendships, participating in activities they enjoy whether it is at a structured recreation activity or free play at one of the many local community events.

Skills for Life

We understand that resilience and confidence builds positive futures. When you ask us to partner with you to support your child’s life opportunities in the way you want, we will focus on your plans, your child’s goals and achievements.

Plan Development and Management

Identitywa can support you with plan development; coordination and implementation of plan supports; and also a range of plan (funding) management options to suit you.
