My Self, My World, My Dreams
Identitywa’s vision is to see all people live with a sense of purpose, a sense of belonging and a sense of well-being. We’re taking another step toward realising this vision by integrating ‘Personal Outcomes Measures’ into the support we provide.
Personal Outcomes Measures originated in Canada through the Council on Quality and Leadership and have been used in support settings for over 20 years.
There are 23 Measures that define quality of life from an individual’s perspective. Divided into three categories – My Self, My World and My Dreams – they cover areas of a person’s life such as personal goals, friends and intimate relationships, natural support networks, daily routines and work.
Coordinator, Linda O’Brien and the Person-Centered Team will guide the roll-out of this approach.
Linda has eight years experience in using the Measures through her work with the Brothers of Charity in Ireland. Linda said, “This approach is designed to support people to have a good life within the ordinary community according their own needs and wishes.”
All Identitywa Coordinators were trained in the use of the Personal Outcome Measures at a workshop in March.
